What do you recommend for stage one prostate cancer

What do you recommend for stage one prostate cancer?
My psa two months ago showed 4.4 Last week, a different lab said it was 6.4 resulting in a biopsy. 3 of the 12 samples showed cancer. This is stage One. Any recommendations on best treatment? I am 74 yrs. old and in great health otherwise. Gleason score is six. Twelve biopsies were taken. Three were positive.

You do not give Gleason score, but for truly low risk disease (PSA < 10, Gleason 2-6, T1-2a primary lesion), observation, radiation, and surgery are all options which are reasonable. Surgery would not be a good option if your life expectancy for any reason was < 10 years. Personally, i would go with radiation. There is no "right" answer. Each path has its plus and minuses. You should see both a surgeon and a radiation oncologist to discuss each option (you have lots of time, no hurry here). Another great option if you live near a tertiary care center (academic institution)- many have a multidisciplinary conference, where pathologists, medical and surgical and radiation oncologist will review your case and render an expert opinion. Recommendations for the “Newly Diagnosed” Prostate Cancer Patient 1. Do Not Panic! Many of us have been down this road before, and there is much to learn before you choose the solution “best for you.” You have had this thing longer than you think, and taking the time to do the proper investigation into your options will not hurt. Since you have posted on the Internet, you obviously have the means to do most of the research that you need to do. There is a wealth of knowledge and experience for you to draw on. 2. Do Not Accept the First Recommendation From the Urologist! (Or any other “first” recommendation) Get second, (or more) opinions from experts in other specialties. Depending on your Gleason score, you have time to figure this thing out! Gleason of 6 or less, take your time and be thorough; Gleason 7 and up, you need to do something sooner than later, but remember that it “is not going to kill you tomorrow,” so you still have time to do a good job of fact gathering. Your PSA value enters into this equation also. This is the time to learn and thoroughly examine ALL your options, because whatever you choose, you will live with the consequences the rest of your days. 3. I recommend that you also join “YANA” (You Are Not Alone”) http://www.yananow.net/ Explore it. (My story is on YANA, under Mentor Experiences/Proton Beam). Study this site carefully; the author, Terry Herbert, is “One of Us.” 4. Consider Proton Beam Radiation Therapy (PBRT), which is only available at five “Centers of Excellence” in the U. S. This is the ONLY treatment (other than “Watchful Waiting” which of course has no side effects) that has the least amount of side effects, with at least comparable results to all other options. Usually you will not hear of this option from other doctors or specialists! Do not allow the fact that the doctor did not mention it prevent you from considering this option along with the others! At the very least, study the records about PBRT and if possible, visit one of the centers for a consultation. Disadvantages: Nine weeks at one of only five centers that offer it. High cost (Medicare and most insurance does cover it). Full disclosure: I believe that PBRT results in the best chance for improved “Quality of Life” during and following treatment, and consider myself a “Proton Beam Advocate!” I completed my PBRT in March of 2007. 5. Get Bob Marckini’s book, “You Can Beat Prostate Cancer …” and read it cover to cover BEFORE you visit your doctor again if possible. There is a wealth of information in this book! I am a 16 year old asian american; I shave everyday because my facial hair grows considerably noticable everyday; i have more hair on my arms and legs than almost every asian in my school; plus my acne isn't pleasant either; do i have too much DHT? will this cause baldness? prostate cancer? what can i do to prevent prostate cancer? i don't really care about the baldness part once cancer is an issue; what can i do to lower my DHT; if i have too much

I always thought cancer was all the same and should all have the same urgency to be treated. What makes breast cancer more important than all other forms?

There are many different kinds of cancer and they all need different types of treatment. Men usually don't develop prostate cancer until they are over the age of 45 but there are exceptions. Breast cancer strikes women as almost any age and is thought to be hormone related. There's presently a big controversy over when women should get a mammogram to detect breast cancer but IMO the sooner the better since it can be very deadly. Prostate cancer is a slow growing cancer inmost cases but still there's the exceptions.

I have just done 40 radiations for prostate cancer and believe it is gone.Some people do not bother to get treated and it grows slow enough to not be their demise. Colon cancer is more vicious and more fatal .A lot depends when it is detected. Statistics say 80% of all men will have prostate cancer by age 80. Your grandpa should have a PSA blood test for prostate cancer and a colonoscopy for colon cancer

depends on the gleason stage and how spread it is for prostate cancer. Lots of people live many years with prostate cancer, some are cured depending on what treatments they qualify for. Colon cancer is worse, but if it can be removed by surgery, you can get cured. If surgery can't get to it, and it is spread, the new drug AVASTIN is really good to keep people from getting spread of cancer of the colon

This depends on a lot of factors. Prostate is one of the slowest growing and spreading cancers, so much so that when someone in their 80's is diagnosed the treatment option is often to do nothing as something else will get them long before the prostate cancer. Without treatment 10 years is not uncommon. With treatment, he could make it to his 100th birthday.

Surgery and chemo. He'll need help with the chemo. I know I had it for breast cancer. I don't know if they radiate that area or not.

There are different options and though they all have side effects, he should look into radioactive implants. They are tiny seeds which do not harm those around him and seem to be better than the surgical resection and/ or radiation. He just needs to have a explanation of all the options and statistics. If he is older like 70 it progresses less quickly.

I am doing research for a friend who is considering having elective radiation after a radical prostatectomy. I am hoping for those who have had radiation done for prostate cancer to share their experience about the whole treatment and what were some of the side effects you experienced.

I had prostate cancer 3 years ago. I had Proton Radiation at Loma Linda Universty Medical Center in Loma Linda California.. Proton Radiation is relatively new, but very successfull.. It has little to no side effects... and about a 90% cure rate. Which is as good if not better than other treatment options. I feel it is the very best prostate cancer treatment available today.. The reason it has no side effects is due to the fact that 90% of the treatment energy is released at the exact point of treatment.. Therefore it does not destroy surrounding tissue and organs like conventional radiation does.. The folks at Loma Linda have been so successful with it that other hospitals are beginning to offer it.. MD Anderson in Houston started in May of this year.. also offering it are hospitals in Indiana, Florida and Mass,... the one at Harvard I think.. I had mine in California.. and would do it again...They have been doing it so much longer.. Lots of Doctors wont recommend it... because they dont know about it, consider it expermental or they dont benefit financially by sending patients elsewhere.. If you are interested, there is a website called Proton Bob... that gives much more information.. be sure and read the testmonial section of former patients,... also there is now a book recently published by a prostate cancer survivor Robert J. Marckini.. the book is titled... YOU CAN BEAT PROSTATE CANCER.. It outlines all treatment options available plus gives pros and cons.. When I was at Loma Linda getting my treatments, I met several patients who had radical surgery that did not work, many who had the seeds that didnt work.. I would be willing to bet that they were all cured by Proton Radiation.,..they were sure sold on it.. There are two types of radiation therapy treatments for Prostate Cancer. I am giving them below with the side effect of these treatments- 1.External beam radiation treatment uses high-powered X-rays to kill cancer cells. This type of radiation is effective at destroying cancerous cells, but it can also scar adjacent healthy tissue. Side effects - * Urinary problems. The most common signs and symptoms are urgency to urinate and frequent urination. These problems usually are temporary and gradually diminish in a few weeks after completing treatment. Long-term problems are uncommon. * Loose stools, rectal bleeding, discomfort during bowel movements or a sense of needing to have a bowel movement (rectal urgency). In some cases these problems persist for months after treatment, but they improve on their own in most men. If you do have long-term rectal symptoms, medications can help. Rarely, men develop persistent bleeding or a rectal ulcer after radiation. Surgery may be necessary to alleviate these problems. * Sexual side effects. Radiation therapy doesn't usually cause immediate sexual side effects such as erectile dysfunction, but some men who've had the treatment have sexual problems later in life. 2. Radioactive seeds implanted into the prostate have gained popularity in recent years as a treatment for prostate cancer. The implants, also known as brachytherapy, deliver a higher dose of radiation than do external beams, but over a substantially longer period of time. The therapy is generally used in men with smaller or moderate-sized prostates with small and lower grade cancers. Side effects - * Urinary problems. The procedure causes urinary signs and symptoms such as frequent, slow and painful urination in nearly all men. You may require medication to treat these signs and symptoms. Some men need medications or the use of intermittent self-catheterization to help them urinate. Urinary symptoms tend to be more severe and longer lasting with seed implants than with external beam radiation. * Sexual problems. Some men experience erectile dysfunction due to radioactive seed implants. * Rectal symptoms. Sometimes this treatment causes loose stools, discomfort during bowel movements or other rectal symptoms. However, rectal symptoms from radioactive seed implants are generally less severe than with external beam radiation.-
